REPAIR TICKET DOCUMENTATION
REPAIR TICKET DOCUMENTATION
The repair ticket helps SwiftFix Copier Repair track service completion so SwiftFix technicians can close out work orders. A completed repair ticket must be submitted for each service visit completed. The following steps must be completed for each repair ticket:
Write the work order number in the upper left-hand corner.
Write the equipment ID for the copier being serviced.
Note the date and time of the visit, along with the technician ID and initials.
Log all replacement part numbers if any parts were swapped out during the visit.
If you service multiple copiers at one site, list the additional unit numbers on the back of the form.
In case of a severe malfunction, incomplete repair, or equipment beyond repair, call your team lead to receive a case number. Record this number on the ticket.
Before leaving the site, have the copier’s owner or a representative sign and date the ticket to confirm the visit was completed.

Question
You are a technician for SwiftFix Copier Repair. According to the instructions shown, you should have the copier’s owner or representative sign and date the repair ticket when:
The correct answer is D.
The last bullet of the repair ticket instructions states, before leaving the site, have the copier’s owner or a representative sign and date the ticket to confirm the visit was completed, which matches choice D.
Choice A is incorrect because logging the replacement part is a separate action and does not trigger the signature.
Choice B is incorrect because the case number is received only in the case of severe malfunction, incomplete repair, or equipment beyond repair and is unrelated to the sign-off step.
Choice C is incorrect because the repair ticket is submitted after the visit is completed and the ticket is signed.
Choice E is incorrect because a copier malfunction prompts a call, not a signature on the ticket.
